Following are the insurance plans that we do not accept. Lot of the plans have very similar sounding 'glorious' names. Read them very carefully and call your insurance when in doubt prior to the services  being rendered.

• Humana Medicare PPO
• Humana Select/Preferred

Patients are responsible to check their coverage prior to the services being rendered. Some services that have widely varying coverage include physical exams, vaccinations to name a few.

Medicare replacement plans generally have coverage very similar to Medicare (regarding physicals, vaccinations, pap and breast exams etc.)

Passport Insurance

Kids with Passport up to 20 years of age are required to have a yearly physical (EPSDT). To encourage compliance, we offer free movie tickets to all Passport Kids after they get this EPSDT exam. This also requires full cooperation from the parents including consent for all the needed labs and all needed vaccination. The child in question will forfeit the movie ticket if there is non compliance or refusal to any component of this exam. We reserve the right to stop this program at any time without notice.
